This certification is for faculty and student volunteers who are participating in a University Sponsored event in which they will be engaging with minors according to the policy. Since the training is optional for faculty and students who do not have access, this form is an alternative to confirm that faculty and students participating in a university-sponsored event have acknowledged that they have read and understood the policy. 

This form should be completed and submitted with the program registration form prior to the event.
